The agreement of this contract indicates that the person submitting the form has full authority to sign for the above-referenced company (if applicable) and accepts personal liability for himself and his/her Co-Angler. I/we further agree to allow Belleville Lake Fest to use my photos in promotional materials. I understand the rules and I agree to abide by them. I also understand that failure to comply with the rules will jeopardize my participation in future Belleville Lake Fest and Belleville Area Chamber Foundation events.
